COVID-19 REPORTING TOOLS

The College of Pharmacy is committed to providing a safe learning environment that meets and exceeds the educational needs of its students. We are dedicated to anticipating student expectations while quickly and efficiently responding to concerns as they arise. Below are three different reporting mechanisms that are constantly monitored and addressed. Additionally, all student clinical participation algorithms will be linked below for ease of use and convenience.

REPORT FORM

Students, faculty, and staff must self-report positive COVID-19 test results or symptoms to TAMU here

COVID-19 ANONYMOUS
FEEDBACK FORM

Anyone can submit an anonymous comment regarding COVID-19 directly to the RCOP Task Force here

COVID-19 HEALTH
ASSESSMENT FORM

Students must complete this form within 24 hours prior to coming to campus but no later than 3PM the day prior
